Payment Information
We will provide to you the day of your exam a HCFA 1500 form and a receipt for your exam. These will be needed in order for you to submit to your insurance. Along with these forms you will need to send a copy of the findings for submission.

Payments are accepted at our front desk and are to be paid in full the day of your exam.
